WESTMINSTER HOUSE - Provides shelter plus care affordable apartment living and supportive services for single mothers and their children. The permanent housing restructures family life after homelessness, addiction or related problems. New Directions, an out-patient chemical dependency treatment program located in the historic Park Slope section of Brooklyn, has developed an integrated clinical approach to treatment that focuses on the chemically dependent individual and their immediate social environment. This approach helps individuals to recognize and change their maladaptive behavior, to understand life stressors as they affect chemical dependence, and to develop better coping mechanisms. It also impacts on the individual's social support system, promoting more constructive reinforcement of sobriety through a focus on the family and social network. Inpatient Behavioral Health Services:
The acute inpatient services offer a team approach to care. Assertive Community Treatment Program (ACT Team):
The ACT Team provides mobile intensive treatment and support to people with psychiatric disabilities. The focus is on the improvement of an individual’s quality of life in the community and reducing the need for inpatient care, by providing intense community-based treatment services by an interdisciplinary team of mental health professionals. The mobile multi-disciplinary team comprised of psychiatrists, nurses, social workers, peer counselors, addiction counselors and rehabilitation counselors. The program offers intensive services primarily Monday through Friday, 9am to 5pm, but provides crisis intervention 24 hours a day, seven days a week. Eligibility for admission into the ACT program is determined through a local Single Point of Access process (SPOA), which subsequently refers consumers to an ACT team based on their catchments area.
Adult Outpatient Behavioral Health Services:
The Adult Service provides treatment designed to minimize the symptoms and adverse effects of illness, maximize wellness and promote recovery.
Patients ages 18 and older receive individualized services for acute or chronic conditions requiring long or short-term treatment. Our services include individual, family, group psychotherapy, medication and crisis intervention.

Comprehensive Addiction Treatment Center (CATC):
The CATC program provides outpatient chemical dependency treatment for men and woman over the age of 18 years old. Patients participate in individual therapy, and daily group counseling.

Harm Reduction/Recovery Program:
This grant funded program specializes in reducing substance abuses and the transmission of HIV /AIDS and other Sexually Transmitted Diseases among people who are HIV infected or at high risk for infection. HIV/ AIDS Care:
Provides a full range of HIV and AIDS-related services, including
- Rapid HIV Testing: Provides free, confidential rapid HIV testing for uninsured patients.
- Harm Reduction: Patients can receive individual and group counseling for Alcohol and Other Drugs (AOD), including low-threshold counseling and education regarding substance use, abuse and harm reduction. The programs focuses on sexual risk reduction, IDU risk reduction, HIV and hepatitis C prevention, as well as medical treatment adherence. 1) Inpatient Psychiatric Services:
Provides a 22-bed unit to relieve the effects of various disorders. Treatments may include: psychopharmacological interventions and management, crisis-oriented family and group therapy, augmentation, phototherapy, and Electro-Convulsive Therapy (ECT) for treatment resistant depression. For individuals 14 years of age and older who are experiencing psychiatric or behavioral problems related to neurological illnesses, we conduct a complete workup at our neuropsychiatric center which may include: MRI, EEG, CAT scan, and/or other neuropsychological tests to assess each patient's condition.
2) Outpatient Psychiatric Services:
- The outpatient services are for those who have achieved some stability, but still require additional therapy for psychiatric symptoms. Our specialty outpatient programs include treatment for: a full range of alcohol or other substance abuse, cognitive behavioral disorders (i.e. panic issues, phobias, obsessive compulsive disorders, and post-traumatic stress), memory disorders, anxiety and depression, schizophrenia, and adolescent suicide and depression.
- The Adolescent Depression and Suicide Program is specifically designed for depressed and suicidal adolescents aged 12 through 20 years and their families. This unique program offers a thorough evaluation, brief treatment, family and group therapy, and psychopharmacological intervention.
- For those with a dual diagnosis of substance abuse and psychiatric disorders, treatment may entail: crisis intervention, rehabilitation, and social development orientations to help patients control triggers so they may achieve their goals.
3) Chemical Dependency Program:
Offers a full range of outpatient services that include assessment, counseling (individual, group, and family), referrals to additional or alternative resources when appropriate, and linkages to community resources. Additionally, they offer the following specialized programs:
- Drinking Drivers Program: Our specially trained counselors work closely with the D.W.I. program of the criminal court system and other legal agencies.
- Support groups for women, seniors, and Spanish-speaking individuals
4) Outpatient Substance Abuse Program:
- Unit I and Unit III: These are methadone maintenance treatment programs. In addition to substance abuse treatment, primary care services, HIV specialty care, nutritional services, mental health services and vocational services are offered onsite.
- New Directions Recovery Center: This is an outpatient treatment program established in 2004 to address the treatment needs of adults with alcohol, cocaine, marijuana, and/or opiate abuse or dependence. The services offered include group counseling, individual counseling, HIV testing and risk reduction counseling, psychiatric medication management, psychotherapy, nutritional services, and Buprenorphine induction and maintenance.

Crossroads for Women – A woman is eligible to enter Crossroads for Women if she is 18 years of age or older, is involved in the criminal justice system, and has a history of substance abuse or dependence. Any woman who has an open case in the criminal justice system or who is under probation or parole supervision may be eligible for the program.
Crossroads for Men – Men 18 years of age or older who have current or history of substance abuse or dependence, and were incarcerated within the past two years are eligible to enroll.

Each client goes through an intake screening before admission. To be accepted they must bring:
A picture ID
A medical insurance card
Medications taken. If seeking a rehab admission, a 30 day supply is required.
Change of clothes. If seeking a rehab admission, a week’s worth of clothing is recommended.
Money for miscellaneous expenses ranging between $20 -$50 to cover telephone calls and store runs. Cornerstone does not provide patients with money for miscellaneous expenses.
If client is being referred from a detox setting the counselor/social worker must fax Cornerstone with the history and physical report, the bio-psychosocial report, a current PPD or chest x-ray results, lab results and a list of all current medications the client is taking. All packets will be reviewed by the CTFN’s medical team for face to face approval.

Families will meet with a Family Assessment Specialist, an experienced Administration of Children's Services (ACS) social worker specially trained to work with adolescent/family issues from a family-centered, strength-based perspectives. It also assists and empowers families by helping them identify options that can be used to create action plans.
Family Assessment Specialists use social work skills to work with parents and youth to highlight and affirm strengths; to identify service needs; and to provide referrals to the most appropriate services that will help families work through their present challenges.
Any family can receive assistance from Family Assessment Program. Families are not required to have an open case with ACS to obtain help.

When a family comes to the Family Assessment Program, the parent(s) and the youth are interviewed both separately and together. This permits each family member to share their individual feelings and experiences. This process also explores how each family member has attempted to cope with their situation. Families can be referred to a variety of services: the Designated Assessment Services (DAS) that are used to defer PINS placements; crisis intervention; mediation; preventive services; family counseling; substance abuse programs; anger management; and other services that a family may request.
